The Homeless Prevention Council (HPC) is pleased to announce it has been named a Supplemental Nutrition Assistance Program (SNAP) Outreach Partner with the Department of Transitional Assistance, becoming the second on Cape Cod and the only one to specifically serve the Lower and Outer Cape community. The SNAP Outreach Program is a joint effort between the DTA and local organizations to better assist the public in understanding their eligibility and applying for SNAP benefits.

Beginning on October 1st, HPC can and will assist neighbors in need to complete SNAP applications, interim reports, and recertification; understand the application process; gather and submit required verifications; remain eligible for SNAP by helping with the recertification process. This program is indicative of HPC’s commitment to the Lower and Outer Cape as their community partner for long-term success.

“Becoming a SNAP Outreach Partner is a natural and logical step for HPC to take as we continue to identify and pursue better ways to serve our community.” Hadley Luddy, Chief Executive Officer

Lower and Outer Cape neighbors in need of support are encouraged to reach out to HPC to get the help that they need as quickly as possible. To be connected with a case manager, please call HPC’s office at (508) 255-9667 or email [email protected]
